Geriatric Care Manager

Eldercare Services is looking for a Professional Geriatric Care Manager (PGCM) to join our Walnut Creek team.

We are leaders in the field of geriatric care. We combine professional Care Management and counseling with Caregiver Services to deliver superior services to older adults, their families, and the professionals involved in their care.

Knowledge of local resources, aging issues, family dynamics, medical issues and dementia is necessary. Professionals will support our caregivers in the home to ensure the highest level of care possible for clients and their families peace of mind.

REQUIREMENTS:

  • Bachelor’s degree in the field of Social Work, Nursing, Human Services, Psychology, or counseling
  • Master’s degree preferred
  • 2 years supervised experience working in a Clinical setting
  • Ability to qualify as a member in ALCA (www.alca.org)
  • Previous triage experience is a plus. Home Health or Hospital Social Work is also a plus

Caregiver – 12 hour shifts

Eldercare Services is looking for Caregivers/ Personal Attendants to assist our clients who may suffer from dementia or mental health issues to live as independently as possible.

We prefer those with experience in mental health services, with a good driving record, reliable automobile, and current auto insurance.

Eldercare Services is the leader in the field of geriatric care. We combine professional Care Management and counseling with Caregiver Services, to deliver superior services to older adults, their families, and the professionals involved in their care.

We require every caregiver to be able to work at least one assignment on a weekend day as we work as a team and share the responsibilities. We offer flexible schedules for individuals who’d like to work full time, or part time as a second career.

This is an opportunity to gain more hands-on clinical experience with the mentorship of clinically experienced managers. You must have at least one year of previous paid experience in caring for an individual who cannot care for themselves.

On-Call Care Coordinator

Eldercare Services, a leader in the field of senior care, seeks a part-time on-call Care Coordinator to join our team.

This position will work from your home. You must have a computer, internet connection, telephone reception, and a private area for confidentiality.

The position will be on-call primarily on weekends: Friday 5pm to Saturday 8am or Saturday/Sunday 8am to 4pm or 4pm to 8am.

Woman_on_phoneWe prefer that you commit to 3 weekends per month at least one-shift. You are paid an on-call stipend, along with an hourly rate for time spent taking calls and working.

We are looking for someone who lives within 30 minutes of our office. Only qualified applicants will be responded to so please carefully review the requirements. We need a candidate who wants this type of work on-going.

This position offers flexibility, ability to learn new skills, and a fabulous team environment. Position will need to train several weeks during the daytime hours and 5pm to 8pm several evenings until proficient.

Support our clients and caregivers by helping to solve scheduling challenges and by calling Caregivers when needed. This position will involve Client Telephone Contact, this is not a hands-on Client Contact position, so previous experience in a staffing/scheduling or Healthcare position is a plus. Previous experience in a triage environment is helpful. Not a recruiting/HR position.

REQUIREMENTS:

  • Bachelor’s degree in a Social Work or health related field or current RN/LVN with clinical experience
  • Must have excellent communication skills
  • Must be detail oriented with excellent follow-through
  • Must be flexible and able to deal with stressful situations in a calm manner
  • Must be computer literate
  • Must have a reliable automobile, valid CDL, and insurance
